Acne, a common skin condition, arises from inflammation within the sebaceous hair follicles. Each follicle houses a tiny hair and several sebaceous glands responsible for producing sebum, an oily substance that normally flows to the skin’s surface. Acne occurs when sebum becomes trapped, leading to clogged pores and bacterial overgrowth.

Acne predominantly affects areas with a high concentration of sebaceous glands, such as the face, scalp, neck, chest, back, upper arms, and shoulders. While it typically emerges during adolescence, it can also appear in adulthood.

One innovative approach to acne treatment involves targeting Propionibacterium acnes (P. acnes), the bacteria responsible for inflammation. P. acnes produces porphyrins, naturally occurring molecules that release free radicals when exposed to specific light wavelengths, effectively destroying the bacteria.

Blue light therapy, utilizing wavelengths between 400 nm and 430 nm, has shown promise in treating mild to moderate inflammatory acne. Studies indicate that bi-weekly 15-minute exposures over four weeks can result in a 60% reduction in acne for 80% of patients, with remissions lasting for several months.

Another laser-based acne treatment involves thermally damaging the sebaceous glands to reduce oil production. Research using a 1450 nm diode laser, administered in four monthly sessions, demonstrated complete acne clearance in the majority of patients six months post-treatment. Biopsies confirmed heat-induced alterations in the sebaceous glands.

A dermatologist noted, “Lasers have long been effective for acne scar treatment, but their ability to manage active acne represents a significant advancement, offering an alternative for patients who haven’t responded well to conventional methods. However, this treatment remains under investigation and may not be widely accessible or covered by insurance.”
