Scalp acne, often manifesting as scalp folliculitis in its milder form, is a surprisingly common condition. While most people are familiar with acne breakouts on the face, chest, arms, and back, it can also occur on the scalp. Many are unaware of this possibility unless they’ve personally experienced it.

Stress is a significant trigger for scalp acne and similar conditions. The condition typically arises when the scalp produces excess oil, often due to infrequent washing or the use of oily hair care products.

Scalp acne can cause intense itching, making it difficult to resist scratching. In mild cases, small, crusty, and sore pustules appear, mainly along the hairline on the upper forehead. The number of pustules can range from a few to many.

A more severe form, acne necrotica miliaris, is characterized by inflamed, larger papules with dark crusts. This type of acne can leave scars resembling chickenpox scars. A rarer and more extreme form primarily affects adult African-American men, although it can occur in anyone regardless of race or gender. This condition involves a combination of large cysts and smaller pustules and papules that can grow considerably in size.

For mild cases of scalp acne, treatment mirrors that of typical acne. Keeping the affected area clean with a salicylic acid-based product is essential. However, these products can cause excessive dryness, so apply them sparingly and only to affected areas, preferably with a cotton ball.

Since oily hair contributes to scalp acne, using a shampoo specifically formulated for oily hair is crucial. Seborrhea-specific hair care products have also shown success. It’s also advisable to review the ingredient lists of all hair care products and make necessary adjustments.

Avoid treating severe forms of scalp acne without consulting a dermatologist or physician.

It’s also important to avoid using products containing benzoyl peroxide on the scalp. Benzoyl peroxide can alter hair color, especially in chemically-treated or color-treated hair, leading to undesirable results.
